*In order to be guaranteed the early-bird registration rate, payment must be received by 4/21/23, otherwise registered attendees will be charged the full conference standard rate.
Cancellation Policy:
Cancellation Policy: If you must cancel your registration for any reason, please notify PHA by April 15, 2023, in writing via email to yourpartner@pahomecare.org. If the notice of cancellation is received prior to this date, PHA will provide a credit, minus a 30% administrative fee. This credit must be used within one year of the date of issue on educational programming. Credit will not be accepted for membership dues or PHA’s Annual Conference. No credit will be given for no-shows or cancellations received after April 15, 2023. Substitutions are permitted up to the day of the event. PHA reserves the right to adjust conference delivery to hybrid and/or virtual.

Attendee Policy: At the discretion of PHA, attendees may be required to follow specified COVID-19 (or other infectious disease-related) protocols, depending upon community circumstances and applicable guidance at the time of the event. Attendees agree to self-screen for COVID-19 or other illness-related symptoms and to not attend the conference if they feel ill.
